What You Provide

To get started, we’ll need:

  • Legal structure details (LLC or Inc.)

  • Owner info and driver’s licenses

  • Landlord contact (if renting space)

  • Insurance certificates (General, Professional, Workers’ Comp)

  • Dishonesty Bond and Power of Attorney documents

What to Expect

Once submitted, licensing can take 90 days or more. CDSS will assign you a license number, then a Licensing Analyst to review your file. We stay with you every step of the way—including making any corrections or additions requested by your analyst. Your only job? Forward us any emails you receive. We’ll take care of the rest.
